Independence 1st offers PATH facilitation

Hello All,

 Let's get the word out.  Independence First in Milwaukee has a project to do Person Centered Plans (PATHs) specifically, in Milwaukee, Waukesha, Washington and Ozaukee counties!  This is a cool opportunity for people who live in these areas. The people to contact if interested in getting a plan completed before July 2011 are:
