The two day learning event is designed for individuals, families, service providers, MCOs, IRIS and County Legacy Programs interested in learning how Community Supported Living assists people to live in their own homes.   You will leave the two day series with practical skills that enhance your ability to create Community Supported Living in your life.   Participants need to commit  to both days of the series

Community Supported Living is customized to support individuals at any level of need for assistance to:

• Live in a non regulated setting and exercise choice in regard to location and appearance;
• Have their name on the lease or mortgage of a home that is not owned or controlled by an exclusive service provider;
• Choose to share a home with another person;
• Choose who is engaged in providing their supports;
• Direct when, where, how and what type of supports are provided;
• Direct and have access to individualized and  integrated community participation.

This event will be separated in time to enable participants to work with the information and learning materials provided on Day One before returning for a deeper exploration the “how to’s and next steps” in developing Community Supported Living. Follow-up assistance from inControl Wisconsin will be available to participants after the event.

$30.00 per person covers registration, meals and training materials on both days.
